The Principle Of PLDD
In this procedure, laser energy is transmitted through a thin optical fiber into the disc. The aim of PLDD is to vaporize a small portion of the inner core. The ablation of a relatively small volume of the inner core results in an important reduction of intra-discal pressure, thus inducing reduction of disc herniation.
Percutaneous laser disc decompression (PLDD) is the utmost minimally invasive percutaneous laser techniquein the treatment of the disc hernias, cervical hernias, dorsal hernias(except for the segment T1-T5), and lumbar hernias. The procedureuses the laser energy to absorb the water within the herniated nucleuspulposus creating a decompression.
The PLDD treatment is performed on an outpatient basis using only local anesthesia. During the procedure, a thin needle is inserted into the herniated disc under x-ray or CT guidance. An optical fiber is inserted through the needle and laser energy is sent through the fiber, vaporizing a tiny portion of the disc nucleus. This creates a partial vacuum which draws the herniation away from the nerve root, thereby relieving the pain. The effect usually is immediate.
PLDD has proven to be safe and effective. It is minimally invasive, is performed in an outpatient setting, requires no general anesthesia, results in no scarring or spinal instability, reduces rehabilitation time, is repeatable, and does not preclude open surgery should that become necessary. It is an ideal choice for patients with poor results in non-surgical treatment.
Herniated disc (slipped disc) is a condition that can occur in any part of your spine, but most commonly in the lower back (lumbar spine). The intervertebral disc consists of an inner “jelly-like” nucleus pulposus, which encased by annulus fibrosus. When the annulus fibrosus becomes weakened, nucleus presses against the annulus, causing the disc to bulge or protrude outward. This is knowns as disc herniation.
Symptoms vary greatly, patient may experience a low backache or no pain at all if the herniated disc is not pressing on a nerve. When lumbar disc herniation occurs, pressuring on one or several nerves that contribute to the sciatic nerve can cause pain, burning, tingling and numbness that radiates from the buttock in the leg and sometimes into the foot.
